Webinar Outline

An Advance Personal Plan is a legal document that is used to plan for what happens if someone can’t make decisions for themselves, for example if the person has an accident or becomes unwell.

Advance personal planning replaces enduring power of attorney in the Northern Territory (NT). It does not replace a person’s will and only applies while the person is still alive.

Advance care planning allows health professionals to understand and respect a person’s future healthcare preferences, for a time when they become seriously ill and unable to communicate for themselves.

All healthcare professionals and aged care workers have an important role to help with planning and ensuring people have choice in their care. Learn how to start the conversation with someone, or respond if a patient or client asks you about the requirements of advance care planning.

As a health professional, it’s important you understand the laws and ethical principles and stay up to date with legislative or process changes.

Learning Objectives

At the end of this session participants will be able to:

  • Describe the key components and legal requirements of advanced personal plans in the Northern Territory.
  • Understand the importance of discussing advanced personal plans with patients and their families.
  • Explain the process of creating, documenting, and integrating advanced personal plans into routine primary care practice.

CPD

*One hour of self-reported CPD.
*As a general guide, one hour of active learning equates to one hour of CPD. It is the responsibility of the individual student to calculate how many hours of active learning have been completed. The hours of active learning estimated above should act as a guide to CPD hours.
