Join the Heart Foundation & the NT PHN in this interactive Primary Care Virtual Roadshow, CVD prevention streamlined – A roadmap to better identify, assess & manage CVD risk in the NT region.

Webinar Format

An interactive Virtual Roadshow bringing a tailored approach of Cardiovascular Risk Reduction from the Heart Foundation in collaboration with NT PHN. This interactive, live virtual workshop will bring together a specialist presentation followed by breakout groups and completed with live audience panel Q & A.

Speakers

Assoc. Prof. Nadarajah Kangaharan completed his undergraduate medical degree and postgraduate cardiology training in Australia.  He has been working as a Cardiologist at the Royal Darwin Hospital, Darwin Private Hospital, and NT cardiac since 2005. He has held the position of Medical Director – in the Division of Medicine at the Royal Darwin Hospital since 2009.   

Dr. Brooke Ah Shay is a GP who has spent the past three years working in Maningrida, the Northern Territory’s largest remote Aboriginal community, but has lived and worked all over Australia. Brooke has a keen interest in remote First Nations health and public health. 

Learning Objectives

At the end of this session primary care health professionals will be able to:

  • Update primary care health professionals on the latest clinical risk assessment recommendations incorporating new evidence on risk factors, imaging and biomarkers .
  • Provide practical advice for the application of Heart Health Checks, including available resources for the general practice team. Participants will be encouraged to systematically embed Heart Health Checks as part of routine care.
  • Provide evidence and behavioral insights on communicating CVD risk to consumers
  • Guidance on the management of high blood pressure and cholesterol, nutrition, and other lifestyle support strategies.
  • Increase knowledge on local referral pathways for risk factor management and review.
  • Support the design and delivery of quality improvement activities to leverage national and local incentives for ease of implementation.

  • GPs and nurses working in rural/remote general practice 
  • Allied health working in rural/remote general practice

Northern Territory HealthPathways offer clinicians clear guidance for assessing and managing symptoms or conditions during the point of care. Pathways include information for referrers and requests to services in the local health system.
