The General Practice in Aged Care Incentive (GPACI) – Thin Markets Funding Grant is now open for submissions of proposals.
NT PHN is inviting interested allied health providers, general practices, and solo General Practitioners (GPs) to submit a proposal to participate in the General Practice in Aged Care Incentive (GPACI) – Thin Markets Funding Grants.
This is a time-limited pilot designed to improve access to high quality and sustainable primary care services for permanent residents of Residential Aged Care Homes (RACHs) in the identified thin-markets regions of the NT.
With up to $600,000 in funding available, providers are encouraged to explore approaches to improving coordination of care for aged care home residents, strengthening multidisciplinary approaches in aged care settings, and demonstrate the need for sustainable service models in the Northern Territory.
The tender period closes at 3:00pm ACST, Tuesday 13 October 2026.
