How we work

We fund services across the NT that support older people to stay well, connected with their community and health services, and independent.

Programs are delivered by local providers and community organisations, to make sure people can access care.

Why this matters

The NT has a growing older population. Older Territorians can often face unique challenges in accessing care due to distance, limited local services and workforce shortages, particularly in remote communities.

Supporting locally delivered services helps older people stay connected to care, maintain independence, and continue living well in their communities for longer. It also ensures support is responsive to local needs and culturally appropriate where required.

What we fund

  • Community-based programs that support physical health, social connection and wellbeing.

  • Services that help older people connect with primary care, aged care and other supports to manage their health and stay independent.

  • Programs that assist older people to understand and access the right services at the right time.

  • Initiatives that strengthen capability across the aged care and primary health workforce, including culturally appropriate care.

  • Targeted support in communities to make programs support the needs of the community.
