The objective of this Request for Tender is to identify suitably qualified providers to deliver the Multidisciplinary Team (MDT) Support Services Program in Alice Springs. NT PHN is seeking providers to deliver group-based support services that complement MDT clinical care and improve health and wellbeing outcomes for people living with diabetes, cardiac disease and/or pulmonary disease.
The MDT Support Services Program provides eligible participants with access to semi-structured group activities that support chronic disease self-management, improve functional capacity, encourage healthy lifestyle behaviours, strengthen peer support, and reduce the risk of preventable hospitalisations. Services will be delivered in collaboration with the MDT System Navigator and other MDT providers to ensure coordinated, integrated care.
