Diabetes Nurse Education Services for Barkly Region

New funding available to address the high rates of
chronic conditions in the Barkly region of the NT.

Background

NT PHN are committed to supporting people to reduce high rates of chronic conditions in the Northern Territory (NT). This tender provides funding for diabetes nurse educators to work with individuals and communities to address the high rates of chronic conditions in the Barkly region of the NT.

NT PHN is seeking applications from diabetes nurse educators to provide a regular visiting diabetes education service to remote community health centres in the Barkly region, including:

  • Elliot
  • Ali Curung
  • Canteen Creek
  • Epenarra
  • Alpurrurulam
  • Tennant Creek GP Clinic

The service will be funded by NT PHN through Medical Outreach Indigenous Chronic Disease Program (MOICDP). The successful applicant will be contracted to deliver the service according to an agreed service plan which outlines the visits for each community. The visits total approximately 75 days or 15 weeks per year. The initial contract length is 6 months with the possibility of an extension beyond June 2024.
