Multidisciplinary Team (MDT) Program has officially launched!

Northern Territory Primary Health Network (NT PHN) is pleased to announce that as of 1 July 2025, the Multidisciplinary Team (MDT) Program has officially launched.

The MDT Program aims to improve access to coordinated care for people living with chronic Pulmonary, Cardiac and Diabetes conditions in Darwin and Alice Springs by reducing out-of-pocket costs for allied health services and support programs.

NT PHN CEO Paul Connolly welcomes the launch of the program “By reducing the financial burden of accessing care and fostering a more connected primary healthcare system, we can help Territorians manage chronic conditions more effectively and live healthier lives,” Paul says.

If you are a general practitioner or work in private practice, you need to be aware of the changes this program brings and how to correctly refer a patient. Our website contains all the necessary information and resources to ensure you have a good understanding of the MDT Program.
