Sunrise Health Service Aboriginal Corporation (Sunrise) are seeking a Credentialed Diabetes Educator to work in and provide excellent healthcare to their remote communities.

What’s the role?
As the successful Credentialed Diabetes Educator, you will be able to provide services to all Sunrise clinics and communities for individual and group services to diabetic patients and families. Being part of a passionate team of preventative and chronic disease health professionals, your role will be based in the wider Population Health Team that includes a range of other clinicians and programs You will travel regularly to the nine remote clinics, with some overnight stays required.
Responsibilities include, but not limited to-
- Providing high quality and individualised diabetes management/care for patients and their families suffering from, or at risk of diabetes and associated conditions
- Working collaboratively with all health centre and specialist staff to ensure appropriate planning and implementation of care for patients receiving care for their diabetes and associated conditions
- Working as a member of the Chronic Disease team and communicating effectively with all disciplines within the health care team
- Providing clinical leadership to other health professionals including Remote Area Nurses (RANs), Aboriginal Health Practitioners, medical and allied health professional staff in the area of diabetes management
- Initiating patient referral to other health professionals and promoting a multidisciplinary team approach to decision making and care provided
What do you need to apply?

- AHPRA registration as a Registered Nurse or Nurse Practitioner and credentialing as a Diabetes Educator
- Extensive professional experience in diabetes and chronic disease management
- Demonstrated understanding of the principals of Aboriginal community controlled health services
- Ability to practice in a sensitive and culturally safe manner in a remote Aboriginal Community
- Current Criminal History check, Working with Children (Ochre Card) and Manual Driver’s License with 4WD drive skills or willingness to obtain
What’s it like living in Katherine?
Located on the Katherine River 312 kilometres south-east of Darwin it is often described as the place where ‘the outback meets the tropics’. Katherine is centre of a large beef cattle region, an important gold producer and offers a range of swimming, fishing and recreational parks. One of the most spectacular features of the Katherine area is the Nitmiluk National Park. This national park includes the Katherine Gorge (Nitmiluk) which can be explored by canoe, boat and helicopter.
