Free CPD Education Evening in Darwin – ‘Advance Personal Planning (APP) and Coordinating Community Palliative Care’
Learn how you can be involved in your patients’ end of life journeys through Advance Personal Plans and Community Palliative Care.
The NT PHN invites Primary Health Professionals to join us for this free CPD education evening at the Oaks Elan Hotel Darwin. A buffet dinner will follow the presentation.
Our presenter, Samantha Harrington, a will guide you through what to know about supporting your patients to develop an Advanced Personal Plan and the best practice for coordinating community end-of-life care.
Who is this for?
We encourage all Primary Health Professionals including GPs and GP Registrars working in primary health settings to attend to learn how your practice can best support your patients to plan for their care in the future. Samantha will dispel the myths around APP and demonstrate how Primary Health Professionals can play an important role is supporting their patients to plan for future care. Contrary to what is commonly believed, most families do NOT know what end-of-life decision their loved one would make. Sam will also provide an overview of the role of GPs in palliative care, including how and when to refer to the specialist palliative care team.

Samantha Harrington
- Samantha Harrington has worked for NT Health across Top End NT since 2001. In 2015 she started working as a Clinical Nurse Consultant with Palliative Care. Sam has many years of experience with Advance Personal Planning and coordinating community palliative care. She’s passionate about educating and empowering health professionals to promote these more widely, to ALL the people they care for.Â
Learning Objectives
APP
By the end of this session, participants will be able to:
- Summarise the legal obligations related to the documentation required when developing an Advanced Personal Plan.  Â
- Identify the target demographic that could benefit from Advanced Personal Planning.Â
- Recognise opportunities to begin discussions with patients about the importance of an Advanced Personal Plan.  Â
- Describe the responsibilities of a Substitute Decision Maker, in the Advanced Personal Planning process.Â
Coordinating Community Palliative Care
By the end of this session, participants will be able to:
- Know when / how to refer to the specialist Palliative Care team
- List the benefits of involving Territory Palliative Care
- Understand the role of GPs and community health service providers with End of Life Care

Using HealthPathways for CPD
RACGP and ACRRM acknowledge HealthPathways as a valuable clinical tool for GPs and have endorsed its use to contribute to Continuing Professional Development (CPD) requirements. HealthPathways provides clinical information for local practitioners and the opportunity to engage in reflective practices to deepen comprehension of patients’ clinical conditions, enhancing overall patient care.
