APNA Online, Intermediate Chronic Disease Management and Healthy Ageing Program (NiPHC) 6-Month Course
Advance your skills in the focus areas of cardiovascular health, cancer control, injury prevention and control, mental health and diabetes mellitus.
This virtual workshop is the first workshop in our 6-month intermediate level Chronic Disease Management and Healthy Ageing Program.
If you are looking for a foundational course please view APNA Principles of Chronic Disease Management which is for nurses to develop and apply skills in chronic disease management, developing foundational knowledge and building practical strategies.
Program objectives
- To enhance understanding of the principles of chronic disease management and healthy ageing for five key chronic conditions.
- To incorporate and apply the concepts of chronic disease management and healthy ageing within nursing practice.
- To identify strengths and areas to grow in relation to chronic disease management and healthy ageing practice.
Induction Workshop Date and Time
Tuesday 26th of Feb time in each state below:
- Victoria: 4-6pm
- Tasmania: 4-6pm
- New South Wales: 4-6pm
- Queensland: 3-5pm
- South Australia: 3:30-5:30pm
- Northern territory: 2:30-4:30pm
- Western Australia: 1-3pm
Monthly Workshops
Following the first induction workshop, each monthly workshop will discuss the role of the primary health care nurse in chronic disease management and healthy ageing care focusing on the key priority areas of:
- Cardiovascular health
- Cancer control
- Injury prevention and control
- Mental health
- Diabetes mellitus

For consistency all workshops will be held from 4-6pm AEDT^. Workshop topics will be advised one month in advance.

*Whilst we encourage participation in the live online sessions, workshops will also be recorded.
^daylight saving time will end on the 5th of April 2025 which will impact the time of workshops for some states throughout the program.
Self Assessment
Upon registering for the workshop, nurses will complete a self-assessment (online survey) to develop of an understanding their current knowledge, skills and confidence and identify any areas for improvement.
Community of Practice
A private messaging platform where program participants can discuss, engage and consult with their peers. APNA provides tips and resources to the group, as well as encouraging and facilitating discussion.
CPD
APNA provides online learning modules of approximately 12 CPD hours to nurses. We encourage nurses to use their self-assessment as a guide through their personalised learning journey. These are optional and nurses are not required or expected to complete all of the learning activities.
