The GP Liaison program is a joint partnership between NT PHN and NT Health designed to:
GP Liaison Officers are experienced GPs who strengthen coordination between primary health services and the hospital system. They use their clinical expertise to improve referral pathways, discharge processes and communication between providers. Working closely with hospital clinicians, general practitioners, Aboriginal Community Controlled Health Organisations (ACCHOs) and other partners, GP Liaisons help patients receive the right care, supporting better outcomes and reducing healthcare gaps across the Territory.
We have 2 GP Liaisons in the NT, Dr Roxane Craig in Darwin and Dr Kelly Banz in Alice Springs. Working across Royal Darwin, Palmerston and Alice Springs Hospitals, they have helped reduce rejected GP referrals and improved coordination through outpatient waitlist management, local specialist education and shared care programs in renal and endocrinology. They are strong advocates within hospitals to improve completion of discharge summaries to facilitate transitions of care from acute to primary care.
