What is the tender?

A grant from NT PHN, valued at $567,000, will be available for works completed between Feb/March 2026 and June 2029. The funding is for projects in the Central Australian and/or Barkly regions that address some or all of the goals of the Greater Choices for at Home Palliative Care (GCfaHPC) Program.

The Greater Choices for at Home Palliative Care program aims to:

  • improve access to the best palliative care at home
  • support palliative care services in primary health and community care
  • make sure Territorians get the right care, at the right time and in the right place to reduce unnecessary hospital visits
  • generate and use data to improve services
  • use technology to provide flexible and responsive care, including after-hours care.

These aims are supported by activities such as:

  • Improving quality and coordination of primary health care
  • Supporting measures that improve the coordination and integration of health services to manage health in the community with a focus on complex and chronic conditions, reducing potentially preventable hospital attendances and admissions
  • Supporting the delivery of health information, advice and services through interactive communication technology to help people care for themselves and their families
  • Support the workforce development, quality improvement and data development activities in the area of palliative care
  • Promote the use of tools and expertise available through the National Palliative Care Projects
  • Support uptake and use of advanced personal plans (APPs).

Frequently asked questions

Capacity-building activites that may be funded

A recent needs assessment identified several activities that fit the goals of the program. Your project may include any of the following:

  • Creating a community of practice to encourage collaboration and shared learning
  • Providing continuing professional development and promoting existing palliative care resources.
  • Working with subject matter experts to deliver culturally safe training and capacity building.
  • Helping clinicians build skills to discuss goals of care and advanced personal plans with patients.
  • Giving patients and families plain-language information about palliative care and what to expect.
  • Embedding discussions about APPs and goals of care into routine care, especially chronic disease management.
  • Improving integration between service providers, keeping the patient’s wishes and needs at the centre.
  • Supporting culturally safe care, including through Aboriginal and Torres Strait Islander staff and governance.

Funding coverage

A single grant is available for the purpose of achieving these aims. This table shows the breakdown of funding available. Activities can start from the February or March 2026.

Successful applicants should:

  • Plan to operate with the core strategic aim of increasing access to quality palliative care at home
  • Plan to build capacity of the health care workforce and or local organisations to provide quality palliative care at home and in the community 
  • Plan to enhance primary care through education or quality improvement initiatives to deliver quality multidisciplinary palliative care 
  • Comply with all child safety obligations 

Funding cannot be utilised for:

  • Clinical service delivery and palliative care service provision.
  • Activities occurring outside of the NT. 
  • Activities that are not sustainable past 30 June 2029 or require ongoing funding.
  • Activities under existing funding mechanisms, e.g. MBS, activity based funding, hospital/NTG funding and other funding avenues.
  • Capital asset purchases.
  • Medication and consumables for ongoing palliative care service delivery.
  • Funding for staff bonuses and cash-like products.
  • Vehicle hire, refurbishment of premises and other activities unrelated to this grant opportunity.

Assessment process

There is a limited amount of funding available for this activity and NT PHN will consider the following criteria in the assessment of applications:

  • Priority and alignment with priority areas (20%)
  • Service capability and relationships (30%)
  • Value for money (30%)
  • Sustainability (20%)
