Funding for early interventions for healthy ageing

EOI for early interventions for healthy ageing, slowing decline, and the ongoing management of chronic conditions in older Territorians.

Background

NT PHN is inviting requests for tender (RFT) to deliver early interventions for healthy ageing, slow decline and the ongoing management of chronic conditions in older Territorians.

NT submissions will be considered with priority given to applications from Big Rivers, Barkly and Central regions.

A total funding pool of $800,000 is available for activities to be started in October 2023 and completed by 30 June 2024. 

Suggested activities in tender scope

  • Co-design and implementation of targeted interventions to prevent, identify and reduce chronic disease and health issues, avoid inappropriate hospital admissions and improve health outcomes for the elderly. 
  • Support collaborative approaches between multidisciplinary teams and primary care providers. 
  • Expand existing healthy ageing programs where relevant. 
  • Educate primary health care providers on how to connect older Territorians with necessary psychosocial, health, social and welfare supports. 
  • Educate family members or carers on how to manage an older person’s health.
